Dive Physical Forms

Commercial diving puts real physical demands on your body — deeper depths, longer bottom times, and heavier equipment than recreational diving. Because of this, every FCDI student must pass a commercial dive physical performed by a UHMS-affiliated hyperbaric physician before their program start date. This isn’t the same as a standard scuba physical; it’s a more thorough evaluation designed to confirm you’re medically fit for the pressures and exertion involved in surface-supplied and commercial diving work.

Below are the required forms: the ADCI Medical History Form, the ADCI Physical Examination Form, and the DCBC Occupational Diver’s Certificate of Medical Fitness. Download and take all three to your physician for evaluation well ahead of your start date — while it’s preferred that this exam be completed prior to admission, delays here can push back your enrollment, and FCDI is not responsible for fees if a student fails the physical.

Once completed and signed off, keep a copy for your records and bring the originals with you on your first day. If you have questions about which physicians are approved, our admissions team can point you in the right direction.
